What is IUI?

IUI or intra uterine insemination is an out patient simple procedure, in which a concentrated sample of morphologically normal and highly motile sperms are placed inside the uterus near the time of ovulation. This sample of sperms is obtained after processing on the semen sample. When is IUI advised to a couple?

Infertility is defined as inability to conceive after 1 year of normal, unprotected intercourse. IUI is a simple, inexpensive, non-invasive technique advised as a first line treatment for reasons such as mentioned below:

  • Unexplained infertility (all reports are normal)
  • Mild endometriosis
  • Cervical disease (sperms cannot pass through mouth of uterus)
  • Ovulatory dysfunction (woman is not having regular ovulation) for example PCOS which is very common
  • Mild male factor (sperm count less than normal but not severely so)
  • Male sexual dysfunction like impotence, ejaculatory dysfunction or hypospadias
  • Sexual incompatibility or vaginismus

When is IUI not effective or should not be done?

  • Blocked fallopian tubes.

As it could be understood, IUI is a ‘blind’ technique which simply involves the deposition of healthy spermatozoa inside the uterus. Eventually, sperms will have to perform their duty of swimming across the tubes in order to meet and unite with the egg for forming an embryo. If the tubes are not patent, it is obvious that the sperms and eggs would never meet. So, IUI would be ineffective.

  • Severe endometriosis - it is better to proceed for IVF in such cases
  • Severe oligospermia (sperm count < 5 million/ml) or Azoospermia ( 0 sperms) - here, there are simply not enough sperms to be able to swim and meet the egg. ICSI is indicated in these cases.
  • If IUI is already done more than 2-3 times before.
  • Now scientific evidence is replete to show that if success with IUI would occur, it would usually occur in the first 3-4 attempts. Going on doing IUI repeatedly is a waste of time and money and should be avoided. Couple should go for ICSI or IVF if they have tried IUI 3 times before and failed.

What is the principle in doing IUI?

In a natural intercourse, the seminal fluid is deposited in the vagina from where the healthy sperms would swim up, go through the resistance of the uterine cervix and finally reach in the fallopian tubes. The female reproductive organs are made in such a way that the cervix would let only normal, healthy, actively motile sperms to pass through. It is kind of a “selection process” where the fittest sperms would be allowed to meet the egg.

What we do in IUI is first we process the semen sample. This processing - which could be done by various methods like density gradient or swim-up technique, essentially removes the seminal plasma and unhealthy sperms giving us a processed sample rich in healthy, actively motile sperms. This sample, which is highly concentrated motile, morphologically healthy sperms is then deposited inside the uterus by a thin IUI cannula by navigating through the cervix. This procedure is painless and there is no need of anesthesia or bed rest. It is done around the time of ovulation so that the sperms and egg can meet and we can have a successful pregnancy.

What is double IUI and should we go for it?

Double IUI means that the doctor is performing IUI procedure two times in the same cycle on successive days. For example, after giving the ‘rupture’ (HCG) injection, if we do IUI at - let’s say after 24 and 48 hours (twice), we have done a double IUI. Whereas, single IUI is usually done only once at 36 hours after the hCG injection. So, which one is better?

Scientific evidence from published papers is very strong now in saying that double IUI has no advantages over single IUI. There could be a scenario of mild male factor infertility (low sperm count) where only probably the double IUI could help. In other words, except maybe mild male factor, doing double IUI is of no benefit. It only increases cost and logistical effort.

How is IUI done?

  • At the best infertility treatment in Delhi NCR, the woman is prescribed an oral tablet like Letrozole with or without gonadotropin injections at around day 2 of her menses. She is asked to come for a few times to the clinic for monitoring of the ovarian response (what we call as Folliculometry scans).
  • After the follicles look mature (at around 20mm size), the doctor prescribes an injection to make the follicle rupture and release the egg. This injection is usually the hCG hormone.
  • At around 36-40 hours of taking the injection, the couple is called to the clinic in the morning.
  • The husband gives his sample and it undergoes semen processing.
  • Processed semen sample is now loaded into a sterile catheter called IUI cannula
  • Wife is made to lie on the table in a proper position and IUI cannula is passed into the uterus and the sperms deposited slowly
  • After 5 minutes rest or so the lady gets up, takes the prescription and goes home
  • The blood test to see for pregnancy is done after 15 days of the IUI procedure.

What are the medicines used in IUI?

  • Along with an oral pill that we will give you from day 2/3 of menses, we would also give you Gonadotropin injections in a low dose till the follicle becomes mature
  • hCG injection is given for follicular rupture
  • After IUI is done, we would usually give you Progesterone tablets and/or vaginal pessaries to be taken at least until the blood test results are back.

What is the success rate of IUI?

  • As mentioned before, IUI is a blind technique. There are factors impacting on the success not in our control, as far as IUI is concerned. For example, we do not know if the sperms would go and meet the egg. We do not know if the tubes will perform their responsibility of making the sperms and egg move and meet. We do not know if fertilisation will occur, and even after it occurs, embryo will come and implant in the uterus.
  • In International journal publications of reproductive science, most papers quote a success rate around a range of 12 to 18% after IUI (per attempt). This is higher compared to a success rate of about 2% per month, if the couple tries only natural intercourse.
  • The success rate depends on many parameters, some of which are the age of female partner, severity of endometriosis, quality of semen parameters including total motile sperm count (TMSC), the number of follicles generated, tubal patency and potency, method of semen processing, and finally the expertise of the doctor/embryologist.
  • It is important to understand that if success would happen with IUI, it is most likely to happen in the first few attempts. If a patient tries IUI for 3-4 times and fails, we usually advocate going for advanced procedure like IVF/ICSI. The success rate of IVF is far higher and in the range of 60-80% per attempt.
  • After the female partner crosses an age of 37-38 years, the ovarian reserve is declining rapidly and IUI would not be a prudent procedure to choose then. We usually advice patients to go for IVF in these cases as IUI success is fairly low in such patients.

IUI is a completely painless procedure. However, some patients experience mild cramping.
Just after IUI is done, we make you lie down for about 10 minutes after which you can get up and go about your normal routine. There is no need for any bedrest nor any special care needed afterwards.
The semen sample is taken from the husband at the hospital itself and it takes about an hour to process it. After the processed sample is ready, IUI will be done within 30 minutes.
We usually do it without anesthesia as it is a painless procedure. However, if the patient is anxious we can offer you short anesthesia or sedation.
IUI is a safe procedure. Some rare complications include OHSS and multiple pregnancy. Twins can happen and depending on medication used, chances of twins is 10% to 40%.
