1. Infertility doesn’t discriminate

Infertility incidence and distribution is similar across all socio-economic, religious and cultural demographics of the society. It does not matter what economic status, occupation, religion or caste you belong to. Even more importantly, men and women have almost equal contribution as seen among Infertile couples. This makes it important that both husband and wife are investigated simultaneously to evaluate their case.

2. The woman’s age is the most important parameter

There is a basic difference between how a man’s and a woman’s body ages over time. A man will continue to produce sperm, albeit at lower count and motility. However, a woman simply has no remaining eggs near menopause. Much before menopause, around the age of 35, the decline of the egg quality in the woman’s body is accelerated. Fertility declines rapidly after the age of 35 for a woman. Around the age 37 to 38, the egg quality is already diminished. It is imperative that if you want to have a baby with your own eggs around this age, you shouldn't wait and waste time.

3. Though you have had a child previously, you can now have problems conceiving

This is called Secondary Infertility and could be due to blocked tubes, advancing age, uterine lesions like fibroids or polyps, impaired sperm parameters or ovulation problems. Again, it is important for you to visit the right IVF or Infertility Centre and have yourself evaluated to know where you stand and what should be done to restore your fertility.

4. Do not wait until it is too late

As mentioned above, nature has made female body in such a way that after a certain age, the fertility comes down. There is no magical elixir or fountain of youth that can restore ‘youthful vigour’ as far as ovarian reserve and egg quality is concerned. If you (wife) are aged more than 35 and you have been trying to get pregnant for more than 6 months, you should visit a good Fertility clinic. 6. There are other treatment options besides IVF

Not all ART (Assisted Reproductive Techniques) is IVF or ICSI. We will determine if you can do away with IVF depending on your age, investigation reports, semen analysis, tubal patency, etc. Safe, cheaper and effective solutions like Ovulation induction or IUI exist that can suffice in many such patients. A step-wise approach can be chosen when IVF will be advised when IUI doesn’t work for 3 attempts.

8. IVF or ICSI does not always mean you will have twins or triplets

This practice can have devastating results like higher order pregnancies that are not viable till birth. 9. When you are facing repeated failures, there’s no harm in trying somewhere else

While trying ART treatment, it is imperative to know that the probabilities of conceiving are optimised by three to maximum six treatment cycles, including the frozen embryo transfers. If your IVF centre has failed to examine a vital data of your case, or you are unable to conceive it might be time to move on to the next best alternative. This is particularly relevant for those couples who wish to have pregnancies out of their own Eggs or Sperms. Very commonly it is seen that an average IVF Clinic would advise to go for donor eggs or donor embryos just because one or two cycles have failed. 10. Knowing which IVF centre to choose is very vital

We have seen that couples distraught by infertility first seek treatment at their trusted source - the family physician or their general Gynecologist. Based on his or her recommendation, they then visit the referral IVF Clinic but find that particular clinic does not cater to their aspirations, or have dismal pregnancy success rates.   1. Foremost, is the doctor qualified by a degree recognised by the Medical Council of India (example FNB)? Most IVF clinics would want you to ignore this fact as they are manned by doctors who self-proclaim to be IVF specialists after just a few months of training!!
  2. Do they have a QUALIFIED, IN-HOUSE SENIOR EMBRYOLOGIST? Most clinics have this unfortunate trend of having only junior staff in the embryology laboratory and the senior ones are “Fly-by” embryologists who’d only come for ICSI and rush off as soon as it is over. Do understand that your precious embryos are made by the Embryologist, who is the single most important person in this whole process of achieving pregnancy. Is the embryologist qualified and have sufficient experience?
  3. How is the technology? Do they have scientifically updated equipments? Do they have a rigorous Quality Control and Quality Assessment (QC & QA) programs running in their labs regularly?
  4. Is the staff compassionate and humane towards their outlook? Do they offer emotional well-being counselling? Do they even care about us like human beings or we are just part of sheer ‘numbers’? Are they a ‘chain’ boasting of many centres across the country who just want to show numbers in the tune of many thousands?
  5. Does the doctor know about or offer Natural cycle IVF / mild IVF? Do they have sequential transfer protocols or segmented IVF to enhance success rates? Are they a dedicated, stand-alone IVF Centre or a busy practising Obstetrician who has developed IVF for some side-income?
  6. Do they offer newest technological advances like IMSI, PICSI, Laser hatching, micro TESE for Azoospermia, ERA, PGS (chromosomal screening of embryos), Time-lapse selection of embryos or single embryo transfer?
  7. Will my embryos be cultured in 20% O2 or 5% O2 bench-top incubators? What is the clinic’s experience in Blastocyst transfer? What is their experience in giving pregnancies out of Frozen Eggs?
  8. Do they provide assurance or confidence about preventing mishaps like ‘embryo switch’ or transferring misplaced embryos of other patient into myself?   10. Last but not the least, what are their success rates? Are other patients at their clinic getting pregnant? Do they mostly offer donor treatment rather than focussing on my own sperms or own eggs? Do they keep offering ‘packages’ for multiple cycles or focus on just one cycle at a time?
