Fastest way to conceive - Reduce the TTP (Time taken to Pregnancy)

One question we most commonly go through is, “Doctor is there a way by which we can reduce the time taken to pregnancy?” “Of course we know if not the first attempt the second or worst case third attempt will lead to a pregnancy, but can we do anything to maximise the success in the first attempt itself?”

Although not simple, let us explain the answer in understandable concepts. In an IVF or ICSI cycle what we do is give injections to the woman to create as many good follicles that would in turn yield oocytes. Not all of these oocytes are mature (Metaphase II). The good, mature eggs are subjected to ICSI and are fertilised. The resulting embryos are cultured till day 3 or day 5 depending on the patient and the situation. Again, not all these day 3 or day 5 embryos would be genetically normal. It is a rule of nature that human embryos are about 30-50% defective genetically. We in medical language call it “Aneuploidies”. Aneuploidy means basically that the number of chromosomes could be 45 or 47 instead of the normal 46. Even if the number is normal there could be structural chromosomal defects or translocations.

Another reason why an IVF cycle fails in defective endometrial receptivity. Even when the embryos are normal a non-receptive endometrium would not be able to execute the process of implantation - leading to embryo wastage and cycle failure. There are many reasons why in a particular cycle the endometrium is not receptive - one of the main reasons is Gonadotropin injections that are given for follicular growth. The ovarian stimulation leads to supra-physiological (abnormally elevated) levels of hormones leading to disturbances in endometrial receptivity.

If we could, somehow, transfer genetically optimal embryos in a receptive endometrium, there is nothing preventing a successful pregnancy in that cycle. So how do we do that?

For making sure that the embryos we are transferring are genetically normal - there is a technology called PGS or PGT-A. As already described in the PGS section, here we take a small biopsy of the embryo cells and send them to a genetic laboratory where the genetic analysis is done. Using the ultra-modern NGS (Next Generation Sequencing) technology, we can easily come to know the genetic status of each embryo tested. Thus, not only we have better embryos that have been successfully cultured till day 5 as Blastocysts, we also have genetically normal embryos confirmed with the PGS method.

For making sure that the endometrium is receptive - we have a technology called ERA or Endometrial Receptivity Analysis (or Array). Here instead of the embryonic biopsy we send a small biopsy of the endometrium to the genetic laboratory. Using specially made algorithms, the laboratory figures out whether the endometrium is good to go (receptive), too early (pre-receptive) or too late (post-receptive). If we transfer genetically normal embryos in a good, receptive, triple-layered endometrium, we don’t see why you won’t get pregnant.
