What is Natural IVF?

In a conventional IVF cycle, we give you Gonadotropin injections to stimulate many follicles for trying to retrieve a good crop (about 12-16 in number) of eggs. Although not harmful at all, there are some effects of high stimulation that include multiple pregnancy (twins and higher order), OHSS and enlarged ovaries. The high stimulation also makes us freeze all the embryos because in a fresh transfer after giving high gonadotropins the results are sub-optimal because of non-receptive endometrium. We call it Natural IVF when no injections are given altogether. This can be done only in normally ovulating women less than 35 years of age. The body’s own natural ovulation is tracked using urinary LH kits and when ovulation is detected the oocyte retrieval is planned accordingly. This approach quickly came out of vogue because not only natural IVF is extremely cumbersome, it has poor results as well. With Natural IVF, we need to continuously monitor the urinary LH kits and when it comes positive we cannot plan in advance. The egg pick-up procedure can come up in the middle of the night also. A much more pragmatic solution to these troubles was the minimal IVF or Mild IVF.

IVF Lite or Minimal IVF or Mild IVF

In the best IVF center in Delhi , to circumvent the problems of Natural IVF, experts came up with Minimal or Mild IVF employing injections but only in a very mild dose. These are usually started on day 2 or 3 of menstruation and are accompanied with some tablets. Monitoring of ovarian response is by blood tests and ultrasound scans just like in a normal or conventional IVF. It is just that instead of targeting to get 12-20 eggs, we are targeting to retrieve only about 3-5 eggs but of good quality. There are many published articles in Journals that indicate that although the yield of eggs is lower in mild IVF, the quality of eggs is better so even when we are getting lesser embryos as a result, they are probably better quality embryos.

Mild IVF is gentler, friendlier and cost-effective. After giving mild doses the few number of eggs are retrieved. This cycle is repeated over 2-3 occasions so that all the eggs/embryos from all retrievals are frozen and pooled together. Once we decide to go for transfer, all these embryos are subjected to blastocyst culture and the best one selected for transfer inside the uterus. We can also go for pre-implantation genetic screening.   • When we go for repeat egg retrievals won’t the total cost be too high?

This is exactly the advantage of IVF Lite or Mild IVF. Since the dose of injections is low, the total cost of the cycle would be only half or even 1/3rd of a usual cycle. This gives us both cost opportunity and time opportunity to go for repeated retrievals, something we call as “Cryo-accumulation”. Cryo-accumulation has given us excellent results to the challenging of challenging cases with poor ovarian reserve and this approach stands as one of Janini IVF’s secret in giving a baby to them. What are the advantages and disadvantages of mild IVF?
